C2 ("Cool Classes") Program Questionnaire
Last year, we kicked off our C2 ("Cool Classes") Program
for our Delta (middle school) students and it was a huge success! C2
classes focus on exploration of real-world subjects in which the kids
have an interest &/or which we feel would be beneficial. Submit this
form if you or someone you know would like to teach a C2 class.

Parent Consent and Field Trip Driver Form
Authorizes AHB to use photos and/or videos of your child(ren) to publicize
AHB. Additionally, this form gives consent to AHB Community School to
transport your child(ren) to activities which are held away from the AHB
campus and asks for driver's license/insurance information if you plan
on helping to transport students.

Authorizes AHB Community School to obtain information relating to parents'
